Data Protection

We need to hold personal information about you on our computer systems and in paper records to help us to look after your health needs, and your doctor is responsible for their accuracy and safe-keeping. Please help to keep your record up to date by informing us of any changes to your circumstances.

Doctors and staff in the practice have access to your medical records to enable them to do their jobs. From time to time information may be shared with others involved in your care if it is necessary. Anyone with access to your record is properly trained in confidentiality issues and is governed by both legal and contractual duty to keep your details private.

All information about you is held securely and appropriate safeguards are in place to prevent accidental loss.

In some circumstances we may be required by law to release your details to statutory or other official bodies, for example if a court order is presented, or in the case of public health issues. In other circumstance you may be required to give written consent before information is released – such as for medical reports for insurance, solicitors etc.

To ensure your privacy, we will not disclose information over the telephone or fax unless we are sure that we are talking to you. Information will not be disclosed to family, friends or spouses unless we have prior written consent, and we do not, leave messages with others.

You have a right to see your records if you wish. Please ask at reception if you would like further details about our patient information leaflet. An appointment may be required. In some circumstances a fee may be payable.

Important Information for Patients

This practice handles medical records in-line with laws on data protection and confidentiality. 

We share medical records with those who are involved in providing you with care and treatment. 

In some circumstances we will also share medical records for medical research, for example to find out more about why people get ill.  

We share information when the law requires us to do so, for example, to prevent infectious diseases from spreading or to check the care being provided to you is safe.

You have the right to be given a copy of your medical record. 

You have the right to object to your medical records being shared with those who provide you with care. 

You have the right to object to your information being used for medical research and to plan health services.  

You have the right to have any mistakes corrected and to complain to the Information Commissioner’s Office. Please see the practice fair processing & privacy notice on the website or speak to a member of staff.

Who we are and what we do

  • Tonyfelin Medical Centre has 12,700 patients in the Caerphilly South Area. We are also a member of the Caerphilly South Neighbourhood Care Network which is made up of 7 separate Practices.
  • Practice details and our opening times are published on our website. 
  • The practice team consists of the practice manager, deputy practice manager, nine doctors, three nurses, one healthcare assistant, one phlebotomist, eight receptionists (care navigators), three administrative staff and one secretary. 

What we spend and how we spend it

  • Tonyfelin Medical Centre receives money from NHS Wales according to its contract for national General Medical Services in exchange for services provided for patients.
  • Financial information is available on request.
  • There may be circumstances where material cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes, under the Act, has taken the view that it may be prejudicial to the conduct of the Practice’s affairs. If this is the case, we will respond to your FOI request with a formal letter acknowledging the reasons why we are unable to give you this information.

What our priorities are and how we are doing

  • We are currently working closely with our colleagues in the Caerphilly South Neighbourhood Care Network to develop and improve services for patients in the local area.
  • Working towards achieving required standards within the Welsh Government QIF (Quality Information Framework), Access Standards, Medicines Management, Patient Safety, CGPSAT, IG toolkit.
  • Performance data can be obtained by writing to the Practice Manager

How we make decisions

  • The Practice has a committee that is made up of all the partners, salaried GP's, practice management and nursing team.
  • The committee meets every month

Our Policies and Procedures

  • All policies and procedures are available for viewing, upon request in writing. Please contact the Practice Manager.
  • If you have a complaint or concerns about the service you have received from the doctors or any of the staff working in this practice, please let us know. We operate a complaints procedure as part of the NHS system. Our complaints system meets national criteria.
  • A copy of our practice complaints procedure is available on our website. This will give you all the information and contact details needed to lodge a complaint.
