Some services will attract a charge. Examples of these are: access to or copies of medical records, holiday cancellation forms, insurance claim forms, certain medicals. The attached list shows the majority of charges. Note that you will be required to pay in advance for any service requested and that the report / letter / certificate will be completed within 1 week.
